Our Commitment to Privacy 

Donor Network of Arizona (DNA) is the nonprofit organization responsible for creating and maintaining the systems to facilitate organ donation and providing public education and outreach programs.  

This Privacy Policy, together with our Terms of Use, govern your use of websites maintained by DNA, including DNAZ.org, DonateLifeAZ.org (the “Registry”), and LiveOnAZ.org (“LiveOnAZ”) (collectively referred to as the “Website”), including all materials, resources, information and services on the Website, whether as a guest or registered donor. This Privacy Policy explains our respect for the privacy of every visitor to the Website and for the privacy of your personal information. This Privacy Policy also explains our information and data security practices as well as to notify and give you options about our collection and use of your information.

Children’s Online Privacy Protection Act Compliance 

DNA complies with the requirements of COPPA (Children’s Online Privacy Protection Act). The Website is only directed to people who are at least 13 years old or older.  

Website Information 

The Website has been designed to provide information to the public, the transplant community, health care professionals and support networks. The Website is not intended to provide or give medical advice, for which a physician should be consulted. 

The Website contains third-party information. Such information is, wherever possible, marked with the name of the source and does not represent our opinion. DNA does not have any knowledge of the information contained in such other sites, nor has DNA been able to include such other sites in its efforts to provide accurate information. We do not take any responsibility for such third-party information. Correct and current information is updated by our webmaster. However, we exclude any warranty, whether express or implied, for any information provided under these pages. We cannot be held responsible for any action taken that is based on the information hereunder. 

Use of Cookies 

Cookies (often known as internet cookies) are text files with small pieces of data — like a username and password — that are used to identify your computer as you use the Website. Specific cookies are used to identify specific users and improve their web browsing experience. Data stored in a cookie is created by the server upon your connection. This data is labeled with an ID unique to you and your computer. Information We Collect 

We collect two types of information:  

  • Personal information. We only collect personally identifiable information from you if you affirmatively choose to provide this information to us.  
  • Standard Web server traffic pattern information. We collect information about the operation of the Website, such as the volume of visitors, what pages are visited, how long visitors remain on the Website and the pages they visit, the type of device and browser used to visit the Website, referring websites, average duration of visit to the Website, and total visitor traffic. We collect IP address information to actively monitor our network for security purposes, allowing us to identify potential threats and maintain a safe online environment for all users. This data is used solely for network administration and is not linked to your personal identity unless required by law to investigate suspicious activity.  

How We Use Your Information 

DonateLifeAZ Registry 

We maintain the DonateLifeAZ Registry. In order to create an account and register as a donor, we require the following personal information: first and last name, street address, city, state, zip code, sex, date of birth and how you heard about donation.  

Personally identifiable information submitted by visitors registering as organ, eye and tissue donors will be added to the DonateLifeAZ Registry and kept confidential. Use of donor information is restricted to those purposes set forth in Arizona law. Specifically, specific individual registry information is used to confirm a person’s status as a registered donor when a hospital makes a report of death or imminent death and with procurement organizations including out-of-state federally designated organ procurement organizations designated by the United States department of health and human services to serve areas out of state to carry out the decision to become a donor.  

To access your registry profile, including to verify or amend your status on the DonateLifeAZ Registry, you will need to enter your first and last name, date of birth, and password or driver’s license number. Registered donors may access their personal profile online at any time to change or modify their information. If your personal information changes, or if you no longer wish to be registered as a donor, you may correct, update or remove your information online. Financial Contributions 

If you choose to make a financial contribution, we require you to enter contact information and standard credit card information (card number, card type, expiration date). We keep a record of your donation but do not store credit card numbers. 

Your Story 

The Website allows you to share your donation story on LiveOnAZ.org, which allows you to add your name or the name of a loved one and to also to share information about how donation has impacted your life. DNA reserves the right to edit information that you submit. Your use of the story sharing feature on LiveOnAZ.org is subject to our Terms of Use. 

Volunteer Information 

If you submit a volunteer application, we may enter your name into our database and contact you in order to discuss your volunteer application and provide you with information regarding our programs, events and services.  

Employment Information 

If you contact DNA about employment, we will have the option to enter your name into our database so that we can respond to your inquiry.   

Aggregate Information 

We may use personal information to generate aggregated information that does not allow the individual to be personally identified or contacted. Aggregated information shows visitor activity as a whole rather than on an individual basis and may be used to improve the effectiveness of the Website.  

Newsletters 

We send periodic messages (such as newsletters, bulletins, educational materials or marketing materials) to individuals who have opted-in to receive them. Recipients may unsubscribe from the newsletter at any time by using the provided unsubscribe link within the email or by emailing ContactUs@dnaz.org.  

Text Messaging Privacy 

No mobile information will be shared with third parties/affiliates for marketing/promotional purposes.  

Legal Requirements 

Other than as set out above, we release personal information to third parties if a law, regulation, search warrant, subpoena or court order legally requires us to do so.  

Security 

We use appropriate security measures (including encryption and other electronic and procedural safeguards) in order to prevent the loss, misuse and alteration of personal information under our control. However, no transmission over the internet can be guaranteed to be completely secure. Consequently, DNA, and our service providers and their respective officers, directors, employees and agents do not represent, warrant or guarantee that personal information will be protected against misuse, loss or alterations.  

Service Providers 

We may use third parties to provide services such as credit card processing, website content management, and email management. In all cases, We work with these would ensure that these third-party service providers to perform tasks on our behalf and are obligated not to disclose or use it for any other purpose except maintain the privacy of your information in accordance with this Privacy Policy. 

Links 

This Website contains links to other websites, which are operated by third parties and which have different privacy practices. We are not responsible for the privacy practices of such other sites. We encourage you to read the Privacy Policy of each web site that collects personally identifiable information.  

Notification of Changes 

Any changes to this Privacy Policy will be posted on the Website to allow you to always be aware of what information we collect, how we use it, and under what circumstances, if any, we may continue to use or disclose it.
